5-Amino-1-methylquinolinium (5-Amino-1MQ) is a potent, cell-permeable inhibitor of nicotinamide N-methyltransferase (NNMT), the enzyme that siphons nicotinamide away from the NAD⁺ salvage pathway and consumes the cellular methyl-donor pool. By blocking NNMT, 5-Amino-1MQ raises intracellular NAD⁺ and S-adenosyl-methionine levels, tipping adipocytes away from lipogenesis toward increased energy expenditure; in diet-induced obese mice, 11 days of subcutaneous dosing cut body weight, white-fat mass, and adipocyte size without reducing food intake while simultaneously improving insulin sensitivity and lipid profiles. More recently, NNMT inhibition with 5-Amino-1MQ has been shown to preserve and even enhance contractile muscle strength in aged, sedentary mice, suggesting exercise-mimetic benefits that could make it a valuable tool for sarcopenia or post-injury recovery studies. Because heightened NNMT activity is implicated in type 2 diabetes, non-alcoholic fatty-liver disease, certain cancers, renal fibrosis, and neurodegenerative conditions, 5-Amino-1MQ is gaining traction as a versatile probe for metabolic, oncologic, and age-related disease research; however, no human clinical trials have commenced.
